What you'll do

• Provide direct care and daily-living support
• assist with hygiene, grooming, meal preparation, and household tasks
• implement individualized support plans and document progress
• transport and accompany clients to appointments and community activities
• administer medications and follow health protocols as directed
• foster independence, dignity, and community inclusion
• communicate with supervisors, families, and care team members.
