What you'll do

• Conduct assessment visits in patients’ homes, nursing facilities, or healthcare facilities
• provide information and admission services
• coordinate with the Referral Department, referring physicians, and hospice physicians regarding referral appropriateness and required certifications
• provide nursing care, emotional support, and education
• document care
• and collaborate with the interdisciplinary team.
