What you'll do

• Direct, organize, and evaluate the social services program
• complete or oversee psychosocial assessments, care plans, progress notes, and discharge documentation
• participate in interdisciplinary care planning
• advocate for resident rights and escalate concerns
• facilitate resident and family meetings and conflict resolution
• respond to grievances and track follow-up
• support advance care planning and end-of-life preferences
• recruit, train, schedule, coach, and evaluate staff when applicable
• participate in quality improvement, audits, surveys, investigations, and corrective plans
• safeguard resident information.
