What you'll do

• communicate with clients, families, team members, and healthcare professionals
• participate in case conferences
• apply infection-control precautions
• coordinate services and report changes to physicians and other personnel
• document care
• supervise, teach, and direct LVNs/LPNs, HHAs, and CNAs
• perform on-call responsibilities as assigned.
