What you'll do

• Coordinate behavioral health, medical, substance use, and community support services
• assist with access to housing, transportation, food, employment, insurance, and social services
• monitor engagement and conduct outreach
• coordinate referrals and discharge planning
• assist with screenings, assessments, and treatment planning
• provide recovery-related psychoeducation
• support medication adherence
• participate in team meetings
• maintain timely, compliant records and confidentiality
• assist with crisis intervention when necessary.
