Concierge / Community Coordinator

The Concierge / Community Coordinator role is onsite in Oklahoma City, Oklahoma. Pay is $17 per hour. What you'll do

• Oversee and implement residents’ Individual Service Plans
• monitor task completion in the EMR and report changes in resident needs
• maintain shift communication logs and documentation
• assist with activities of daily living as needed, including hygiene, dressing, ambulation, transfers, toileting, nutrition, and medication reminders
• keep care lists current
• train, supervise, and motivate care team members
• review resident needs at least monthly
• welcome residents, families, and visitors.
