• Coordinate services and supports, provide education and coaching, review medications, connect members with providers and community resources, document care, and escalate quality-of-care concerns.
• The role requires approximately 50–75% travel for face-to-face member visits.

Why this role

Fluent Spanish and English reading, speaking, and writing are required. The schedule is Monday–Friday, 8:00 a.m.–5:00 p.m.; training is remote via Microsoft Teams for approximately six weeks. A bachelor’s degree in social work or a related field is required; nurses are not eligible.
