What you'll do

• Coordinate caregiver programs, outreach, enrollment, engagement, resource navigation, education, support groups, Memory Cafés, and community partnerships. Serve as a nonclinical contact for caregivers
• maintain program records and calendars
• track service delivery, outcomes, and grant deliverables
• support reporting and reimbursement documentation
• and coordinate with internal teams and community partners.
