What you'll do

• Conduct psychosocial assessments within five days of admission
• identify needs, interventions, and goals
• participate in interdisciplinary group and quality improvement meetings
• document clinical records
• evaluate responses to social work interventions
• coordinate visit schedules with case managers
• support discharge planning
• assess family coping
• consult with the health team
• and help patients use family and community resources.
