Clinical Coordinator and Quality Assurance Nurse

What you'll do

• Participate in internal audits and clinical documentation reviews
• coordinate audit and survey readiness
• support safety, quality, and efficiency improvements
• participate in quarterly QAPI initiatives and implement PIPS
• review visit utilization and recommend improvements
• reinforce accurate clinical documentation
• track readmission data and analyze high-risk cases
• support preventive action plans
• provide clinical guidance and case consultation
• participate in case conferences and interdisciplinary reviews
• conduct home visits as needed
• train, orient, and mentor clinicians through joint field visits.
