What you'll do

• Meet with assigned individuals at least monthly to monitor requested services
• travel within the Texas Panhandle service area
• participate in annual Person Directed Plan development
• connect individuals with community resources
• advocate for safety and well-being
• complete timely documentation and manage caseloads independently.
