What you'll do

• Provide one-on-one patient care
• assist with bathing, dressing, grooming, toileting, hygiene, feeding, ambulation, transfers, and mobility
• support meal preparation, hydration, and nutrition
• maintain a safe, clean, comfortable home environment
• follow care plans with home health nurses
• observe and report changes or concerns
• communicate with patients, families, and care team members.
