What you'll do

• Provide medical nutrition therapy and individualized care plans for pediatric, adult, prenatal, and geriatric patients
• support chronic disease management and food-insecurity needs
• lead or oversee group nutrition and lifestyle medicine visits
• develop workflows, protocols, screening tools, and patient education materials
• train staff
• participate in quality improvement and outcome tracking
• collaborate with community partners to connect patients with food resources.

Why this role

The schedule begins with eight weeks at 40 hours per week, then transitions to 32 hours per week for full-time pay; tentative hours include two 8 a.m.–5 p.m. days and two 11 a.m.–8 p.m. days.
