What you'll do

• Assist with medication care and administration
• identify residents’ ongoing needs through assessment and Personalized Service Plans
• document and follow through on changes in clinical care needs
• support the Wellness Nurse with monthly wellness visits and service plans
• report medication or service-level changes to the Wellness Director or Executive Director
• assist with monthly weights and vital signs and resident care as needed.
