What you'll do
- Complete initial and ongoing assessments of in-home veteran clients’ physical, psychosocial, and environmental needs
- assess conditions and document observations
- perform evaluation tasks and staff supervisory visits
- educate clients and families on home care strategies and recommend helpful devices or tools
- complete timely care documentation
- and communicate with clients, families, and team members.
