What you'll do

• Coordinate and monitor services
• complete and update individual assessments
• ensure support plans and required plans are accurate and implemented
• coordinate medical appointments and financial supports
• complete health risk screenings and quarterly reviews
• train and supervise assigned Program Specialists
• participate in the assigned POD on-call rotation.
