What you'll do

• Coordinate preliminary and comprehensive resident assessments, including CAAs, triggers, and plans of care
• ensure initial assessments are completed within 14 days of admission
• coordinate resident interviews
• develop and update assessments and care plans quarterly, annually, and after significant changes
• evaluate resident conditions and medical data
• ensure appropriate health professionals participate.
