What you'll do

• Engage, assess, monitor, educate, coordinate, and provide interventions for participants and families
• conduct community and home visits as appropriate
• facilitate group education
• contribute health information to multidisciplinary teams
• support treatment plans and electronic health record documentation
• coordinate referrals
• provide backup for specified medical assistant tasks.
