Community Health Worker

Job overview

The Community Health Worker works in a partly office-based and partly community-based arrangement in Cottage Grove, OR, including client homes and other community locations. What you'll do

• Build supportive client relationships
• help clients find primary care and access health and social services
• provide one-on-one therapeutic support using appropriate mental health strategies
• monitor needs and progress against treatment plans
• schedule appointments and assist with applications
• attend appointments when needed
• support health goals and care plans
• coach clients on chronic condition management and self-care
• complete documentation
• coordinate with treatment teams and community partners.
