Hospice Registered Nurse — Community-Based Care

The Hospice Registered Nurse role is onsite in the community in the Town of Tisbury, MA, serving patients across Martha’s Vineyard. The schedule is Monday–Friday, 8:30 a.m.–5:00 p.m.; compensation is $85,000–$97,000 per year. The role supports Hospice and Palliative Care of Martha’s Vineyard, a nonprofit hospice serving the island community. The nurse provides comfort-focused hospice and palliative care in patients’ homes, assisted living communities, and skilled nursing facilities. The role contributes to coordinated end-of-life care through assessments, individualized care planning, patient and family education, and collaboration with an interdisciplinary team.

What you'll do

• Provide hospice and palliative nursing care in homes and care facilities
• complete nursing assessments and individualized care plans
• coordinate with physicians, social workers, chaplains, and hospice team members
• educate patients and families on symptom management, disease progression, medications, and end-of-life care
• document care.
