What you'll do

• Collaborate with physicians and other professionals to plan care

• visit patients as needed to evaluate their condition
• provide support and comfort according to individual needs
• work with caregivers on care
• help administer medication
• alleviate sudden end-phase symptoms
• notify a doctor when necessary
• assist patients and families with paperwork
• maintain accurate records
• and provide support to relatives coping with loss, including post-mortem support.
