What you'll do

• Assess patients’ psychosocial needs related to illness, environment, and support systems
• develop and implement individualized care plans
• counsel and support patients and caregivers
• help navigate resources, financial concerns, and care planning
• coordinate referrals and liaise with community agencies
• collaborate with nurses, physicians, and interdisciplinary team members
• participate in case conferences and discharge planning
• maintain timely electronic medical record documentation and communicate updates.
