What you'll do

• Provide chronic care coordination under physician and nurse practitioner direction
• collaborate with patients, families, caregivers, and providers
• offer health coaching, education, and referrals
• coordinate care and address condition changes promptly
• support transitions home after hospitalization or skilled-facility stays
• identify eligible patients and coordinate program admission with timely documentation.
