The nurse assesses high-risk and chronically or acutely ill patients, coordinates patient-centered care plans, and helps align treatment with patients’ values and quality-of-life wishes. Through collaboration with patients, families, providers, and community partners, the role supports continuity and appropriate care.

What you'll do

• Assess patient health status and barriers to optimal health
• identify care gaps
• develop and monitor care and self-management plans
• coordinate patient and caregiver participation and education
• conduct face-to-face visits as needed
• communicate with primary care teams
• facilitate referrals
• document care planning and outcomes
• conduct end-of-life discussions.
