Community Living Supports Coach

What you'll do

• Provide instruction and assistance with service-plan goals and independent living skills
• participate in goal and program planning
• document daily activities and progress
• supervise medication administration and monitor health concerns
• transport and supervise clients as needed
• run groups or provide one-to-one support
• record client funds
• support health, safety, and community engagement
• coordinate emergency response with on-call, case management, or emergency providers.
