The Clinical Liaison Teach Nurse role is onsite at Brigham and Women’s Hospital in the Woburn, MA area. Compensation is $47–$52 per hour, with quarterly commissions after the initial training period and a weekend-shift differential if applicable. New England Life Care is a nonprofit home infusion provider serving hospital systems across New England. The nurse assesses whether referred patients and caregivers can safely manage home infusion, contributes to patient-specific training and care planning, and coordinates information between the hospital and NELC. This work supports continuity of care as patients transition from hospital to home services.

What you'll do

• Assess patient and caregiver needs and abilities
• participate in patient-specific training plans and care plans and provide training when appropriate
• facilitate discharge by relaying orders and documentation
• act as a clinical resource between NELC and hospital departments
• coordinate with other home health agencies
• monitor patient status and communicate updates
• document interactions
• communicate patient requirements and changes
• participate in daily care meetings and case conferences
• maintain patient confidentiality
• complete therapy-specific reimbursement determination checklists.

Why this role

Associate degree or diploma in Nursing required; BS in Nursing preferred. Intravenous therapy, oncology, CRNI, BLS, and current RN licensure are preferred. Valid driver’s license and excellent driving record required.
