What you'll do

• Provide personalized care following each resident’s service plan
• assist with bathing, dressing, grooming, toileting, eating, transfers, repositioning, and walking
• record vital signs, weights, and intake/output as directed
• report condition changes to nurses
• document care
• coordinate with the care team
• engage residents in activities
• follow infection-control and facility guidelines.
