• Develop and maintain referral relationships with healthcare providers and other referral sources
• increase home health admissions and meet monthly sales goals
• manage territory plans
• educate referral partners about services
• coordinate patient transitions from hospitals or facilities to home
• collaborate with clinical leadership
• maintain CRM records
• represent the company at networking and community outreach events.
