Social Work Technician

What you'll do

• Complete and document social work and psychosocial assessments within three days of admission or readmission
• ensure required forms and resident care conferences are completed and coordinated
• complete MDS sections and applicable CAAs
• review and update care plans
• monitor behavior logs and high-risk residents
• coordinate with psychiatric and ancillary providers
• educate residents and families about advance directives and discharge rights
• document interactions in the medical record
• support resident adjustment
• coordinate safe discharge planning, DME, and home care.
