Hospice Community Liaison

Job overview

The Hospice Community Liaison role is onsite in Antioch, CA. Base salary is $60,000–$80,000 per year, plus OTE commission. The liaison supports Madrona Hospice Care’s referral relationships and coordination with its intake and clinical teams. The role develops ethical, compliant referral relationships and helps community healthcare partners understand hospice services and referral processes. By promptly coordinating referral information, the liaison helps the clinical team evaluate and respond to potential referrals; clinical eligibility and admission decisions remain with qualified clinical and medical personnel.

What you'll do

• Develop new hospice referral sources and maintain existing relationships
• visit healthcare facilities and community partners
• educate referral partners about hospice services and referral processes
• facilitate appropriate referrals to intake and clinical teams
• coordinate referral documentation and communication
• manage territory plans and account pipelines
• maintain activity and referral records
• follow applicable privacy and marketing compliance requirements.

Why this role

Expected field activity is approximately 7–10 meaningful professional contacts per business day unless management directs otherwise. A bachelor’s degree and relevant hospice, home health, post-acute, hospital, healthcare sales, or business-development experience are preferred. Regular travel throughout the assigned territory is required.
