Case Manager

The position reports to a Unit Director and provides community-based support, including support at the Street 2 Feet Homeless Day Center as needed. The role works with participants, clinical and medical professionals, staff, and community partners to support recovery, independence, and well-being.

What you'll do

• Participate in a multidisciplinary team
• help develop and review recovery plans
• provide community-based case management focused on education, resource coordination, community integration, and plan goals
• complete assessments, documentation, and reports
• build relationships with community agencies and providers
• advocate for participants navigating services
• and provide support at the Street 2 Feet Homeless Day Center as needed.
