What you'll do

• Provide screening, crisis intervention, and evaluation or assessment for individuals affected by behavioral health conditions
• refer patients to appropriate levels of care
• develop and maintain a community referral network
• complete required documentation and data collection
• coordinate care with Prairie Ridge services, including psychiatry and peer support
• maintain knowledge of reporting and electronic health record systems
• attend consultation, supervision, and training.
