What you'll do

• Develop, manage, and grow referral relationships with hospitals, skilled nursing facilities, rehabilitation centers, senior centers, physicians, case managers, discharge planners, and social workers
• reactivate inactive referral sources
• conduct in-person outreach and sales conversations
• generate qualified referrals
• track referral-to-tour, assessment, and move-in outcomes
• partner with sales and executive leaders to improve conversion
• support community events and professional open houses
• maintain CRM records and report referral activity and results.
