What you'll do

• Conduct outreach to patients and families
• assist with appointments, referrals, and support services
• build relationships that support engagement and treatment-plan adherence
• identify barriers such as transportation, insurance, food insecurity, and housing
• provide culturally appropriate health education
• support care management and clinical teams with follow-up
• document interactions in the EHR
• participate in multidisciplinary meetings, training, and quality improvement
• process and track referrals.

Why this role
