What you'll do

• Complete admission and comprehensive assessments and update plans of care through recertification
• case manage assigned patients through symptom management, ongoing visits, and timely care plan updates
• support and educate patients and families
• participate in weekly interdisciplinary team meetings
• coordinate care across the team
• communicate with facility staff and referral partners
• document in the Consolo EMR
• participate in rotating weekend on-call coverage.
