The Registered Nurse (RN), Home Health role is onsite in Fond du Lac, WI, with travel within a one-hour radius. Pay is $38.50–$44.80 per hour, based on experience; higher pay may be considered depending on qualifications and experience. The role is part of CommonHeart’s Home Health and Hospice team. The RN assesses and meets patients’ nursing needs, coordinates care plans, and provides education and support to patients and families. Documentation, communication, and collaboration with the care team help ensure quality home health services.

What you'll do

• Assess patient and family needs
• manage medications and documentation
• coordinate implementation of care plans
• participate in care coordination and quality improvement
• provide patient and family education and counseling
• supervise other nursing personnel
• respond to patient and family calls
• practice infection control
• and provide on-call services as assigned, including the stated rotation.
