What you'll do

• Develop Individual Family Service Plans with families and identify short- and long-term services
• secure services in the plan and help families access services from other sources
• monitor and evaluate service delivery
• arrange alternatives when services are unavailable
• counsel individuals and families about progress and treatment
• provide health-care education when possible
• maintain records and prepare reports.
