What you'll do

• Deliver support per individual service plans.
• Assist with personal care, housekeeping, and recreational activities.
• Administer medication and monitor health.
• Facilitate community integration and transportation.
• Document progress and activity accurately.
• Maintain healthy relationships with individuals and their families.
