What you'll do

• Provide direct care to older adults across clinic, long-term care, assisted living, and home settings
• conduct comprehensive geriatric assessments
• diagnose and manage chronic conditions and medications
• develop treatment plans and educate patients and caregivers
• guide end-of-life planning and palliative care coordination when appropriate
• collaborate with the multidisciplinary team
• support clinical protocols, case reviews, provider training, quality improvement, and accurate documentation.
