MDS Coordinator

What you'll do

• Complete and submit timely, accurate MDS assessments
• select appropriate ARDs and diagnosis codes for PDPM classification and reimbursement
• monitor PDPM components and validate supporting documentation for CMS compliance
• coordinate and lead care planning across Nursing, Therapy, Social Services, Dietary, and Activities
• conduct and participate in daily PDPM, weekly CMI, Utilization Review, monthly Triple Check, and care plan conference meetings
• use listed assessment and documentation programs
• perform other duties as assigned.
