Clinical Care Coordinator

What you'll do

• Maintain confidential treatment records
• coordinate care and community-agency relationships
• train and evaluate staff
• assess consumers through interviews and evaluations
• oversee crisis plans, assessments, referrals, and treatment plans
• ensure timely assignment to Community Support Workers
• provide at least 10 billable services weekly
• support crisis intervention during work hours and after hours as assigned
• coordinate aftercare
• review documentation and service quality
• support Medicaid applications and recertifications
• ensure compliance with applicable standards.
