What you'll do

• Coordinate support to reduce treatment barriers
• screen potential clients, schedule intake appointments, and facilitate orientation
• assist with intake documentation and insurance paperwork
• conduct wellness check calls and maintain progress notes
• coordinate care with primary care and other providers
• arrange transportation as needed
• facilitate transfers and support voucher applications.
