Center Case Manager

The Center Case Manager is an onsite role at Pūnāwai Rest Stop in Honolulu, HI, paying $24–$27 per hour. Mental Health Kokua supports people experiencing homelessness and behavioral health challenges through its community services. The case manager builds trust with guests and helps them pursue stability and recovery by assessing needs, developing practical goals, connecting them with housing, healthcare, benefits, and community resources, and coordinating support with colleagues and partners.

What you'll do

• Engage with guests respectfully
• help individuals access housing, healthcare, benefits, behavioral health services, and community resources
• complete assessments and person-centered goals
• provide crisis support and de-escalation
• collaborate with nurses, case managers, and community partners
• advocate for guests, assist with applications, and maintain accurate documentation. Requires a high-school diploma or equivalent, screening, CPR/First Aid certifications, and TB clearance. Schedule: Monday–Friday, except holidays, 7:00 a.m.–3:30 p.m., 40 hours per week.
