What you'll do

• Identify high-risk patients and establish education touchpoints
• provide transitional care after admissions or emergency-room visits
• manage referrals and connect patients with health-plan services and community resources
• interact with patients by phone and in person
• triage acute needs and coordinate with providers on orders and treatment plans
• assist providers with hands-on care when needed
• schedule appointments
• document accurately and protect patient privacy
• review provider scorecards and identify improvement opportunities.
