What you'll do

• Provide personal care and assist with activities of daily living according to the plan of care
• maintain a safe environment
• assist with ambulation, transfers, and range-of-motion exercises as directed
• prepare nutritional snacks
• perform applicable wound, skin, and bowel care protocols under nurse direction
• report changes in patient status
• document care and observations
• attend care conferences, meetings, and in-services
• assist the Team Leader with designated activities. Home-care medication assistance and on-call rotation are specified for Home Care only.
