The professional provides individualized daily support at home and in the community. The role contributes to clients’ well-being by assisting with daily living, documenting care, and communicating with supervisors, colleagues, and families.

What you'll do

• Provide direct care, supervision, and companionship
• assist with bathing, grooming, meal preparation, medication administration, and light housekeeping
• support community outings, appointments, and recreation, including transportation as needed
• maintain a safe and clean environment
• complete daily notes and incident reports
• follow agency policies and applicable regulations
• communicate with supervisors, team members, and families
• maintain confidentiality and professionalism.
