The Personal Care Assistant (PCA) / Client Care Resource Specialist role is an onsite, part-time position serving clients across the Twin Cities Metro Area, Minnesota, with flexible scheduling based on employee availability and agency needs. Pay is $19–$21 per hour. Grace n’ Care is a Minnesota home care provider focused on dependable, compassionate, person-centered care that promotes independence, dignity, and quality of life. The role provides care across multiple client assignments to support new client start-ups, temporary coverage, and continuity of care. It assists clients with daily living needs, documents care and changes in condition, and travels between assignments as needed.

What you'll do

• Provide person-centered care according to each client’s care plan
• assist with activities of daily living, transfers, mobility, meals, and feeding as needed
• perform light housekeeping related to care
• provide companionship and emotional support
• assist with authorized outings and appointments
• observe, document, and report changes in condition
• complete Electronic Visit Verification documentation
• support new admissions and temporary coverage
• travel between client assignments across the Twin Cities Metro Area
• maintain client confidentiality and agency compliance.
