Case Manager – Fontaine Inn Emergency Shelter

Job overview

The Case Manager role is onsite in San Jose, CA, with compensation of $27.75–$33.83 per hour. The schedule is Sunday–Thursday, 1:00 PM–9:30 PM, and driving is required. PATH’s mission includes helping people experiencing homelessness find housing and access supportive services. The case manager provides individualized support to shelter participants as they work toward housing and personal goals. Through assessment, service planning, referrals, documentation, and crisis support, the role helps clients access resources and build pathways toward stable housing and greater independence.

What you'll do

• Conduct intake and individualized needs assessments
• collaborate with participants on Individualized Service Plans
• provide referrals, information, and support
• complete program assessments and 90-day reviews
• maintain timely, accurate participant records and confidentiality
• build professional relationships
• provide crisis intervention and risk assessment
• use motivational interviewing and trauma-informed care
• maintain knowledge of community resources
• attend required training, meetings, and events.
