What you'll do

• Remain available by telephone and email during assigned shifts
• conduct urgent physical and psychosocial assessments
• provide nursing care and skilled pain and symptom management
• guide patients and families regarding diagnosis, medications, and treatments
• document care and clinical changes
• communicate with physicians and obtain orders as required
• collaborate with the interdisciplinary group, vendors, hospitals, and community residential staff
• conduct after-hours hospice admissions as needed
• attend required meetings and continuing education.
