Nurse Discharge Planner

What you'll do

• Collaborate with DOC Re-entry Services on discharge planning for patients with mental health issues or serious medical illnesses
• review external-agency transfer requests and coordinate necessary services
• maintain relationships with community agencies
• communicate with the Medicaid Coordinator about resuming benefits
• advocate for medical necessity and continuation of prescribed medications
• attend relevant meetings and participate in staff training
• consult leadership on complex cases and facilitate resolution.

Why this role
